Procedures

  1. The applicant shall obtain the approved application form from the Reception Desk and fill it out.
  2. The applicant shall then submit the form and the enclosed documents to the Reception Desk officer.
  3. The form and the enclosed documents will be verified for completeness.
    1. If the submitted documents are not complete, the applicant will be asked to update the required documents.
    2. If the submitted documents are complete, the applicant will receive a payment voucher showing the due amount.
  4. The auditor endorses the voucher.
  5. The applicant pays the prescribed fees at the cashier’s counter.
  6. Submit the form and the enclosed documents to the Reception Desk officer
  7. The Applications Intake officer enters data.
  8. The auditor receives the application and validates information against it.
    1. In case of errors, the application is sent back to the Applications Intake officer to double check the entered dat
    2. In case the application is complete, the Audit and Posting officer will post the application.
  9. The application is sent electronically to printing department.
  10. The new card is printed and handed over to the Reception Desk officer.
  11. The Reception Desk officer then gives the new health card to the applicant.

Required Documents

  • A recent letter from the blood bank stating the date of blood donation.
  • A copy of the Family Book.
  • One (1) personal photograph.
